Care Coordination
Care Coordination is provided through the NYS Health Homes program and is specifically designed to help children, families and adults connect with an array of services to address all of their needs; physical health, behavioral health, education and social supports. Individuals and families will have one point of contact, their Care Manager, who provides guidance and support with accessing services to stay healthy, succeed at school and remain involved in their communities.
Adult Care Coordination
AURORA’s Adult Care Coordination provides individualized supports to ensure that adults and seniors who are Deaf, blind, visually impaired or hard of hearing can access needed services and manage their health care needs. Our Care Managers create open lines of communication and facilitate connections among service providers to avoid unnecessary duplication and improve the health and well-being of those we serve.
